Health Requirements
Illinois School Code requires students to submit a report of physical, dental, and vision examinations and the dates that certain immunizations were received in order to attend school.
Ages of interest/circumstances include:
- Preschool/early childhood
- Kindergarten
- First entry in Illinois school
- Grade two
- Grade six
2025-26 Health Requirements for Admittance to School
A student’s parent(s)/guardian(s) shall present proof that the student received a health examination with proof of the immunizations against, and screenings for, preventable communicable diseases, as required by the Department of Public Health. Unless an exemption or extension applies, the required health forms must be presented to the district by October 15 of the current school year in order for a student to attend school.
