Health Requirements

Illinois School Code requires students to submit a report of physical examination and the dates that certain immunizations were received in order to attend school.

Ages of interest/circumstances include:

• Preschool
• Kindergarten
• First Entry in Illinois School
• Grade Two
• Grade Six
