Residency Forms

Anyone seeking to enroll a student must complete the General Residency Form (English | Spanish) and provide the required supporting documentation. Additional forms may be required.

Lack of fixed, regular, adequate residence	School social worker will facilitate completion of the Homeless Student Identification Form (Form #8) with the family and coordinate services.
Own/rent other property outside of district	Residency Form for Families Owning and/or Leasing More than One Residence (Form #5) \| Spanish
If principal or secretary suspect non-residency or need to verify information or documents provided	Principal uses Home Visit Form (Form #6) to conduct a home visit

Foster parent	Director of Special Education Dr. Anne Bowers (630-795-6830 or studentservices@woodridge68.org) acts as a liaison to facilitate the enrollment and transfer of records of students in the legal custody of the Illinois Department of Children and Family Services when enrolling in or changing schools.
